Why should I screen for breast cancer?

According to The American Cancer Society, over 200,000 women will die from breast cancer this year. The average women’s chances of getting breast cancer is 1 in 8.

What is wrong with mammography?

Currently mammography is the most used test to determine breast cancer. According to radiologyinfo.org, “The effective radiation dose from a mammogram is about 0.7 mSv, which is about the same as the average person receives from background radiation in three months. Federal mammography guidelines require that each unit be checked by a medical physicist every year to ensure that the unit operates correctly.” They also state, “False Positive Mammograms. Five percent to 15 percent of screening mammograms require more testing such as additional mammograms or ultrasound. Most of these tests turn out to be normal.”

What is Thermography?

Over 800 studies on breast thermography exist in peer reviewed medical literature. According to Dr. Nicholas LeRoy, “Infrared breast thermography is based on the fact that blood vessel activity surrounding a developing cancer is almost always higher than in normal breast tissue. With an ever increasing demand for nutrients, cancerous tumor cells release chemicals that open existing blood vessels as well as create new ones. Vascular alterations resulting from cancer frequently create temperature changes on the surface of the breast which can be demonstrated with infrared thermography. Thermal abnormalities identified with infrared imaging are among the earliest signs of a pre-cancerous or cancerous lesion of the breast. The ability of infrared thermography to identify these vascular abnormalities and the resulting temperature changes are well-established in research trials.”

He also states, “Obtaining thermal images of the breasts provides valuable information. This is especially true of women with large, dense, fibrocystic or augmented breasts, which are usually difficult to image with mammography. Mammography is dependent upon the density and the size of a breast mass and any additional dense breast tissue makes interpretation difficult. Because thermography is not in any way impacted by density, cysts, or breast size (it is looking at blood flow), it is an extremely useful diagnostic tool to evaluate and manage these women. For all women, a thermogram is like an infrared fingerprint of the breast. It will not change over time unless there has been an alteration in blood flow. It is for this reason that thermography is ideal not only for breast cancer screening, but also for monitoring suspicious findings identified with other tests such as ultrasound, mammography or with physical examination.”

Top Ten Tools for the Physician’s Office

After visiting with hundreds of physicians in the Chicagoland area, I have noticed the practices that are consistently busy, with high patient loads have a few things in common. This list is my observation of the best diagnostic tools used in physician offices for nutritional analysis.

  1. The Saliva Test from DiagnosTechsDiagnosTechs are the originators of saliva testing based in Kent, Washington. When used with Dr. Janet Lang’s Protocols, is some of the most successful practice building tools out there.Example: the eleven sample extended female hormone panel.

    According to DiagnosTech’s Website, “The FHP™ is a simple, non-invasive test. Eleven saliva samples are collected during specified time periods throughout the menstrual cycle.

    The FHP,™ for the first time in routine medical practice, will allow both patient and doctor to actually view the profile of Estrogen and Progesterone simultaneously. Diagnostic guesswork is minimized, and hormonal balance evaluation and customized treatment becomes a reality with the FHP.™In addition, we report the cycle average for Testosterone and DHEA.The expanded eFHP™ also includes 5 FSH & LH measurements to see if brain control and stimulation of the ovaries is optimal.

    Advantages of the FHP™

    1. One or two blood determinations or a 24 hour analysis of urine for these hormones cannot give an idea of the ovaries’ hormonal productivity. The FHP™ gives a good approximation of the ovarian capacity over a whole cycle.
    2. Convenience: requires no blood draws, therefore no repeated clinic visits and avoids the inconvenience of the 24 hour urine collection.
    3. Economy: the fee for the 11 sample test is less than that of 2 blood determinations or a urine analysis for Estrogen and Progesterone.
    4. Physiological Accuracy: research has demonstrated that the free hormone fraction predominates in saliva. Hormones can be found free or bound to protein. The free hormone fraction is very important to ascertain because it is the bioactive fraction that most significantly influences living cells. The salivary female hormone levels correlate at 93% with the free hormones in the tissues.”
  2. The Hair Analysis from Analytical Research Labs Analytical Research labs founded by Dr. Paul Eck should be used in conjunction with Dr. Janet Lang’s Hair Analysis Worksheet. According to Dr. Lang, Supplement recommendations given on the initial report should be ignored.According to Analytical Research Lab’s website, “We are one of only two laboratories that do not wash the hair at the laboratory. Washing hair samples at the laboratory has been shown to erratically wash out the water-soluble elements. This creates much less accurate results. In a 2001 study in the Journal of the AMA (Seidel, S. et al., Assessment of Commercial Laboratories Performing Hair Mineral Analysis, JAMA, 285(1) Jan 3:67-72.) the two laboratories that did not wash the hair, of which we were one, showed superb reliability.”They also stated, “Providing a mineral blueprint of one’s biochemistry, a hair tissue mineral analysis can provide pertinent information about your metabolic rate, energy levels, sugar and carbohydrate tolerance, stage of stress, immune system and glandular activity.”
  3. The Symptom Survey FormThe Symptom Survey Form has 224 commonly found symptoms arranged in 9 groups. The groups include: Sympatetic, Parasympathetic, Sugar Handling, Cardiovascular, Liver/Gallbladder, Digestion, Endocrine-Hyperthyroid, Endocrine-Hypothyroid, Endocrine-Hyperpituitary, Endocrine-Hypopituitary, Endocrine-Hyperadrenal, Endocrine-Hypoadrenal, Foundational Issues, Male or Female. You are asked to rate your symptoms based on frequency/severity. You are also asked to list five main complaints you have, in their order of importance. You can download the English version here.   4. The Case History Form by Dr. Rob DesMaraisThe Case History Form is top down analysis of a patient’s symptoms. The form comes with a key. The amount of information derived from this form is great and can be used in conjunction with the Symptom Survey Form for cross-analysis. The form seems to be formulated from the blue section of the Clinical Reference Guide with many personal insights added from clinical experience.
  5. The Compiled Notes on Clinical Nutritional Products by Dr. Walter H. Schmitt, Jr. I have listed this book because it seems to be a staple of many qualified health care professionals. This book is the work Dr. Walter Schmitt Jr. It is a compilation of notes from the hundreds of hours of nutritional seminars and clinical work he has done.Many practitioners call it “The Blue Book” because of the blue cover. It covers: Glandular Tissue, Common Vitamin Information, The Liver, Gallbladder, Cholesterol, B vs. G Vitamins, Calcium, “E” Vitamins, Phosphorus, Gastrointestinal Tract, Iodine, Calming Nutrients, Aqueous Glandular Extracts, The Citric Acid Cycle and Energy Production, Essential Fatty Acid metabolism, and Vitamins Every Medicine Cabinet Should Contain.
  6. EAV Testing Also called Meridian Stress Assessment or Electro-Dermal Screening.Electro-Acupuncture According to Voll (EAV) “is a method of assessing energy imbalances in the body based on electrical impedance measurements taken at specific acupuncture points. This measurement can provide a practitioner with information on potential issues in specific areas of the body,” according to BioMeridian Corporation. They also state “The actual MSA process is completed by applying a small electrical current to the specific acupuncture-point and creating a circuit by having the patient hold a brass hand-mass. A scale of 0-100, which was invented by Dr. Voll, is still used to assess the electrical response of the point, with Voll noting that a reading of 50 is ideal, above 50 indicating stress or inflammation, and below 50 indicating weakness or degeneration.”
  7. The Zinc Tally Taste Test from Standard Process, Inc.The directions for this simple test is as follows, Put two teaspoons of the Zinc sulphate solution in a cup. Have your patient hold the solution in the mouth for 10 seconds, then expectorate. Ask the patient if there was any taste. If so, ask them to describe the taste they experienced. Patients with severe deficiencies usually do not taste the solution.According to Dr. Bruce Bond, “Zinc is an essential trace element. It is one of the most important trace elements needed by the body. Of the many hundreds of protein enzymes present in the body which allow its chemistry to work, zinc is required by over two hundred of them. Thus, it can easily be seen how a deficiency of zinc can affect so many different functions.Zinc is also important in the production of hormone like substances called prostaglandins (PGs).

    PGs are vital to numerous body functions including:
    • the function of the immune system
    • the expression and control of inflammation
    • skin and wound healing
    • functions of the heart and cardiovascular system
    • absorption of various minerals
    • body temperature control
    • the important functions of fertility, conception, and delivery of the infant.

    The way in which zinc influences PG production is by supporting the function of the enzymes which permit the conversion of the polyunsaturated fatty acids into PGs. The action of Zinc is very direct in stimulating the normal production of PGs thereby maintaining or restoring normal health and fitness.”

  8. The Postural Hypotension Test or commonly known as The Ragland’s TestThe Ragland’s Test is a method of measuring a hyper or hypo state of the adrenal glands.The Procedure:
    1. Take the systolic blood pressure with the patient recumbent.
    2. Let the air out of the cuff, then have the patient stand, then pump up the blood pressure cuff again and promptly take the systolic reading. (Promptly because the test measures a transitory event.)

    In normal patients the second systolic reading is 6 to 10 mm higher. In hypoadrenia there is a failure to rise, or a drop in the systolic pressure.

  9. The Tissue Calcium Cuff TestMuscle cramping at rest is a primary indicator of tissue calcium deficiency. This test is a means of quantifying tissue calcium status. If a deficiency is identified, various forms of calcium and calcium co-factors are tested orally to determine which nutrients will be the most effective for increasing tissue calcium.The Procedure: Place a blood pressure cuff on the patient’s calf. Instruct the patient to let you know when they feel THE ONSET OF a sharp, cramping type pain.A muscle that can withstand 200 millibars is considered to have sufficient calcium stores. A patient who has cramping prior to 200 millibars of mercury is considered to be deficient in tissue calcium. The optimal tissue calcium level is considered to be 240 millibars or more.If a deficiency is indicated, have the patient taste the different forms of nutrients until the pain threshold is sufficiently increased. In many cases the needed nutrients will be a co-factor rather than a calcium source. Ask your local Standard Process Representative for the Checklist and Instructions.
  10. Salivary and Urinary pH Testing.Digestion is a complex process which is controlled by several factors. pH plays a crucial role in a normally functioning digestive tract. In the mouth pH is ideally about 7.5, very weakly alkaline. Saliva controls pH in this region of the digestive tract. pH in the stomach is very acidic and inhibits the breakdown of carbohydrates while there.The strong acid content of the stomach provides two benefits, both serving to denature proteins for further digestion in the small intestines, as well as providing non-specific immunity, retarding or eliminating various pathogens. In the small intestines, the duodenum provides critical pH balancing to activate digestive enzymes. The liver secretes bile into the duodenum to neutralise the acidic conditions from the stomach. The mucosal tissue of the small intestines is alkaline, creating a pH of about 8.5, thus enabling absorption in a mild alkaline in the environment.

    Normal Oral pH
    7.4 - 7.6 (adult)
    7.6 - 7.8 (child)
    A lower than normal oral pH is a sign of Essential Fatty Acid (EFA) need.

    To Increase Oral pH:

    • Support fat metabolism as needed.
    • Provide EFA supplementation
    • Improve dietary fats and oils
    • Improve ability to increase absorption
    • through improved biliary function

Unraveling the Form

The symptom Survey Form can be deciphered in two ways. The first involves the Clinical Reference Guide. A tally key is used to add up the totals from the various sections. The section with the most tallies is considered “primary” and the group with the second most tallies is considered “secondary.” To properly diagnose the patient’s case, care should be given to percentage of tallies in the individual groups and their relationship with the five main complaints. Care should also be given to individual products and combination products with the goal being “the most bang for the buck!” You can download the instructions and key here.

Making Cents of the Standard Process Sales Tax

Why am I now charged sales tax at Standard Process, Inc?

Before we begin, consult your tax adviser as this article should not be construed as advice. A new policy has recently begun at Standard Process, Inc. based in Palmyra, WI involving sales tax for businesses outside the State of Wisconsin. I have done some research to figure out why this change has been made and what steps as a business owner you should follow in order to best protect yourself and your assets.

What is the Streamlined Sales & Use Tax Agreement (SSUTA)?

According to Jessica Iverson, Communications Officer of the Wisconsin Department of Revenue, (608) 266-2300, “Administrator Diane Hardt of the Department of Revenue’s Income, Sales & Excise Tax Division on February 20, 2008 provided testimony on the Streamlined Sales & Use Tax Agreement (SSUTA) to the Senate Committee on Tax Fairness and Family Prosperity chaired by Sen. Robert Jauch (D-Poplar). Hardt’s testimony was in support of conforming Wisconsin to the provisions of the agreement, which has been included in the past two budget proposals by Governor Jim Doyle.

“The Streamlined Sales and Use Tax Agreement is an important part of our overall effort to simplify the state’s tax system,” said Revenue Secretary Roger M. Ervin. “Leveling the playing field for Main Street businesses, while encouraging voluntary tax compliance among all businesses, greatly benefits Wisconsin.”

SSUTA is an effort to simplify the system by which state and local sales and use taxes are administered. The agreement allows states to make independent decisions about which products and services are taxable or exempt, but provides uniform definitions, administrative procedures, and technology standards across all states.

The goal is to increase tax compliance among all businesses, especially multi-state businesses, while removing the competitive disadvantage for local businesses. SSUTA also seeks to address the fiscal concerns related to e-commerce growth.”

What does this mean to my business outside Wisconsin.

Because of the efforts of the Multi-state SSUTA effort (40 states and the District of Columbia banded together to simplify their sales tax codes in order to make sales tax collection easier), Standard Process is choosing to comply.

Standard Process must charge the sales tax of your individual state if the health care practitioner does not provide Standard Process with the required sales tax information. They must comply to remove the competitive disadvantage for smaller local companies. States rely on sales tax for 1/3 of their annual budget. States like New Hampshire and Montana who have no personal tax revenue depend on sales tax even more.

What is sales tax?

According to the Illinois Department of Revenue, “Sales tax is imposed on a seller’s receipts from sales of tangible personal property for use or consumption. If the seller (typically an out-of-state business, such as a catalog company or a retailer making sales on the Internet) does not charge Illinois Sales Tax, the purchaser must pay the tax directly to the department. Illinois Sales Tax has three rate structures – one for qualifying food, drugs and medical appliances; one for items required to be titled or registered and another for all other general merchandise.”

What about “Qualifying food and drugs?

According to Illinois Department of Revenue, “Qualifying food applies to food not prepared by the retailer for immediate consumption, such as grocery store food items. “Qualifying food, drugs and medical appliances” include:

  • food that has not been prepared for immediate consumption, such as most food sold at grocery stores, excluding hot foods, alcoholic beverages and soft drinks;
  • prescription medicines and nonprescription items claimed to have medicinal value, such as aspirin, cough medicine, medicated hand lotion and fluoride toothpaste; and
  • prescription and nonprescription medical appliances that directly replace a malfunctioning part of the human body, such as corrective eye wear, contact lenses, prostheses, insulin syringes and dentures.”

How can I make sure I am getting the best tax rate?

Standard Process because of the “Supplement Facts” label on the back of every bottle, is listed in the category of “Dietary Supplements” no matter what ingredients are in the product. They now charge in aggregate tax of 1% (for Illinois) on total charges (product plus shipping) if you cannot show proof of reseller status. Standard Process then pays the 1% to the Illinois Department of Revenue. They pay it blanketly and do not mention any individual doctor.

The idea is to properly tax your patients and register your business so if you get audited, you have a statue of limitations. If you are not properly set up as a retailer, the Department of Revenue can come back and fine you with interest from when you set up your office. Even if you purchase only a few hundred a year in product, it can add up.

To be safe, you should get an Illinois Business Tax Number. As a reseller, you should add sales tax based on your state and county to products that you sell your patients. You will then pay the Department of Revenue (in Illinois it is Form ST-1) monthly, quarterly, or annually based on volume, which the department determines how often a return must be filed. The 1% (for Illinois) tax from Standard Process will do you no good if you get audited, you will still need to show that you charged your patients the percentage based on the county you do business in.

To take advantage of “Qualifying food and drugs” sales tax rate:

  1. Find out what county you are in.
  2. Go to your individual state’s department of revenue.
    • for Illinois click here.
  3. Find out if you qualify by contacting your tax attorney.
    • “Qualifying food and drugs” have a separate percentage based on your county; for instance Cook County in Illinois is 2.250%; Kane, Lake and McHenry County are 1.750%
  4. Register your business and obtain a reseller number.
    • Register your business to obtain reseller status. The fastest way is to use the Illinois Business Gateway for Business Registration here.
    • Registered resellers in Illinois should pay the Department of Revenue with Form ST-1 which is not available online.
    • To show that you comply to Standard Process In Illinois, it is Illinois Certificate of Resale Form CRT61. This form verifies your reseller status.
    • If your business is already registered and paying sales tax on items sold out of your office, you may already to eligible.

If you qualify, fill out the CRT61 and fax it to Standard Process, you now must charge your patients the appropriate tax rate, keep records of these transactions and pay the appropriate sales tax to the Department of Revenue.

Offer BioMeridian Scans in Your Office

I’ve recently introduced a new service featuring the Meridian Stress Assessment tool from BioMeridian. This service provides a licensed health professional utilizing the BioMeridian machine to visit your office to evaluate the nutritional status of your patients. This fascinating tool determines a patient’s electro-stimulus response based on various acupressure points. The machine will then analyze the various systems within the body and detect any compromised area. This service gives you the obvious benefit of the test results and product recommendations without the financial commitment of buying a machine outright.

How the program works:

A practitioner must call one of our technicians to schedule a day to visit their office. The office will set appointments, up to 15 per day, with its patients to be evaluated with the BioMeridian unit. The appointment will generate a nutritional protocol for each patient and the tech will review the findings with the patient. The patient proceeds to visit the office staff where they place the order for the necessary product and reschedule with their doctor. All reports generated by the evaluations are given to the contracting practitioner.

The fee for the service is $85.00 per patient. The doctor is free to charge the patient any reasonable fee for the test and evaluation. The contracting office is responsible for the ordering and sale of the Standard Process product.
